Clinician-Guided Documentation
Before we can complete or submit any mental health-related disability or FMLA paperwork, we ask for two teletherapy sessions with one of our licensed clinicians. These sessions are required to establish care and clinical context.

- Pay an out-of-pocket fee per document requested
- Be an active client with insurance or private pay
- Participate in any follow-up sessions if updates or new documents are required
What This Includes
This service is available to clients needing mental health documentation for FMLA, Short-Term Disability (STD), or Return-to-Work or Leave Extensions.
- Completion of an official biopsychosocial disability report
- Coordination with your agency or case manager
- One additional document (either a return-to-work or extension form)
- Support in understanding what's needed and where to submit
If additional documents are requested, each will require a supplemental fee and — in some cases — a follow-up session. We'll guide you through every step.

No, ongoing therapy isn’t required—but it’s highly recommended. If you stop sessions and later request an extension, we may not have up-to-date information to submit on your behalf.
You’ll need to complete a new therapy session before we can issue any return-to-work documentation. This helps ensure the form reflects your current mental health status.
Without recent sessions, we may not be able to provide the clinical updates your employer or agency needs. This could result in delays or difficulty getting additional documentation approved.
We’ll work with you to rematch you with another qualified clinician at no additional cost.
The documentation fee is non-refundable once the first therapy session has been completed, regardless of whether your disability is approved.
We provide the necessary clinical documentation to support your application, but we can't guarantee approval from employers, agencies, or disability providers. Our goal is to help you present the strongest case possible.

The clinical team is led by Clinical Director Tiffany Graham LCSW MCAP QS, a certified EMDR specialist, alongside Faatimah Dreher LMHC, Jodi Greenblatt LMFT, and Mark Novak LMHC.
Advanced Client Care accepts Humana, UnitedHealthcare, Aetna, Cigna, Blue Cross Blue Shield, Florida Blue, AvMed, Oscar, Medicare, Optum, Lucet, and BHS, and offers FSA, HSA, and Afterpay options.
